Group Outing Request Form

The Sport Center • 1141 W. Irving Park Road, Schaumburg
The Sport Center is the perfect field trip location for your day camp or school class trip.

Cost: $8 per participant (minimum 20 participants). There is a minimum requirement of one adult/counselor for every ten children/campers (10:1).

Includes:
- Up to four (4) hours at The Sport Center between 9 a.m. and 4 p.m. on a weekday.
- Your choice of a variety of sports and games on our basketball/volleyball courts or soccer fields.
- Reserved space for your group in our meeting/party room for lunch.

A $100 initial deposit is due upon booking; the remaining is due the day of your group outing. Due to scheduled programming, some areas, including concessions, may not be available during your outing.
